Does Insurance Cover ABA Therapy in Massachusetts? A Parent's Guide to MassHealth and Private Plans
In most cases, yes. Massachusetts law requires most state-regulated private health plans to cover the diagnosis and treatment of autism, including ABA therapy, with no age or dollar caps. MassHealth covers medically necessary ABA for children under 21. The main exception is a "self-funded" employer plan, which follows federal rules instead, though many of those plans choose to cover ABA anyway.
If you're starting to look into ABA for your child, insurance is probably one of your first questions. That's a good instinct. Knowing how coverage works up front saves time, avoids surprise bills, and helps you choose a provider with confidence. This guide walks through the basics in plain language.
What is ARICA, the Massachusetts autism insurance law?
ARICA stands for An Act Relative to Insurance Coverage for Autism. Massachusetts passed it in 2010. It requires private health insurers in the state to cover the diagnosis and treatment of autism spectrum disorder, and ABA therapy is one of the covered treatments.
Massachusetts is one of only a few states whose autism coverage has no limits on age, cost, or how often services are provided. For families of young children, that means coverage is there for an evaluation, for therapy, and for as long as care remains medically necessary.
ARICA applies to:
-
Plans bought through Massachusetts insurers, including HMOs
-
Plans for state employees and retirees
-
Small business and individual plans, including plans bought through the Massachusetts Health Connector
Which plans don't have to follow ARICA?
Some large employers pay for their employees' health care directly instead of buying an insurance policy. These are called self-funded plans, and they're governed by a federal law (ERISA) rather than Massachusetts law. They aren't required to follow ARICA.
Here's the good news: most self-funded plans in Massachusetts cover autism treatment anyway. To find out which kind of plan you have, ask your employer's HR or benefits team: "Is our health plan fully insured or self-funded, and does it cover ABA therapy?"
Does MassHealth cover ABA services?
Yes. MassHealth isn't governed by ARICA, but it covers medically necessary ABA services for children with an autism diagnosis until they turn 21. Coverage runs through your child's MassHealth plan, and the ABA provider requests approval (called prior authorization) before services begin.
MassHealth can also help in two other ways:
-
As secondary insurance. If your child has private insurance first and MassHealth second, MassHealth may cover the copays and deductibles for ABA that your primary plan covers.
-
Through the Children's Autism Medicaid Waiver. Families on MassHealth with children under age 9 can apply for this program through the Massachusetts Department of Developmental Services (DDS).
What does insurance usually require before ABA starts?
Every plan has its own process, but most follow the same general steps:
-
A diagnosis. ABA coverage generally requires an autism diagnosis from a qualified clinician, such as a psychologist or developmental pediatrician.
-
An ABA assessment. A Licensed Applied Behavior Analyst (LABA) gets to know your child and recommends a plan of care.
-
Prior authorization. Your ABA provider sends that plan to your insurer for approval, including the number of therapy hours recommended.
-
Reauthorization. Insurers review progress periodically, often every six months, to keep coverage in place.
It sounds like a lot, but your provider should handle most of this paperwork for you.
What will I pay out of pocket?
That depends on your plan. Coverage doesn't always mean zero cost. Depending on your plan, you may owe:
-
A copay for each session or each day of service
-
Your deductible, until it's met for the year
-
Coinsurance, a percentage of each bill after the deductible
Ask your provider to verify your benefits before the first appointment, and ask for an estimate of your monthly costs in writing.
How much does an autism evaluation cost with insurance?
Because ARICA covers the diagnosis of autism as well as treatment, a diagnostic evaluation is generally a covered benefit for families with state-regulated plans, and MassHealth covers evaluations too. What you pay out of pocket depends on your plan's copay, deductible, and coinsurance, the same as for therapy. Some plans also require a referral from your pediatrician first.
Before you book, ask the evaluation provider to verify your benefits and give you an estimate. At ANNA, we do this for every family before the first appointment, for evaluations and for therapy.
